(4) Inquiries may be made by mail using the "Application for Disclosure of Personal Information" form to the contact address below.

(5) If you wish to request notification or disclosure of the purpose of use of your personal information, please pay a handling fee of 1,080 yen per request in the form of postage stamps.

(6) Personal information provided on the "Application for Disclosure of Personal Information" form will be used to contact the customer who made the inquiry. It will not be used for any other purpose.
